Dutch biotech Revealis Biotherapeutics launches to advance TEIPP cancer vaccine targeting immune-escaped tumors
New company builds on first-in-human clinical proof-of-principle and is developing a next-generation, multi-target TEIPP vaccine for immunotherapy-resistant cancers.
Therapeutic cancer vaccines are gaining renewed momentum. Following positive Phase III results from Moderna and Merck with a personalised mRNA cancer vaccine, a different vaccine strategy developed in the Netherlands is moving into its next phase. Newly established biotech company Revealis Biotherapeutics will further develop the TEIPP vaccine approach, which is designed to target tumor cells that have escaped conventional immune recognition.
The TEIPP vaccine was developed by researchers at Leiden University Medical Center (LUMC) and Oncode Institute. In a first Phase I/II study in 26 patients with metastatic non-small cell lung cancer, the vaccine induced a TEIPP-specific immune response in more than 80% of evaluable patients. The findings, published in Nature Communications, provided the clinical proof-of-principle to advance the technology.
Renewed Momentum for Cancer Vaccines
Therapeutic cancer vaccines have been investigated for decades, but many earlier candidates struggled to translate an immune response into clinical benefit for patients with cancer. New insights into anti-tumor immunity, better targets, combinations with immunotherapy and the timing of treatment are changing that perspective.
On 19 August, Moderna and Merck announced positive Phase III results for their personalised mRNA cancer vaccine in combination with pembrolizumab in high-risk melanoma. Revealis Biotherapeutics uses a different technology: the TEIPP vaccine targets a specific mechanism through which tumor cells escape the immune system.

Targeting the Tumor Cells that Become Invisible
One of the major challenges in immunotherapy is that tumor cells can adapt. Some tumor cells alter their antigen presentation and as a result become almost invisible to the T cells that should attack them. These tumor cells eventually drive renewed tumor growth, while current immunotherapies may no longer work effectively. The technology behind Revealis Biotherapeutics is designed to target these escaped tumor cells and restore immune control.
Scientists led by Sjoerd van der Burg and Thorbald van Hall discovered that these “invisible” tumor cells simultaneously display specific TEIPP antigens. These arise precisely when the normal mechanism by which tumor cells present tumor antigens is disrupted.
The immune system does not naturally mount a strong response against TEIPP antigens. The vaccine teaches the immune system to recognize these TEIPP antigens and aims to turn a tumor escape mechanism into a vulnerability.
The first clinical study showed that this principle also works in patients: more than 80% developed a TEIPP-specific immune response following vaccination.
From scientific insight to company
Revealis Biotherapeutics will develop a next generation of the vaccine containing multiple TEIPP peptides. This is intended to generate a broader immune response and make the technology suitable for a larger group of patients.
For this first development phase, pre-seed financing has been brought together by Oncode BV and two family offices. Revealis Biotherapeutics will require additional capital and collaboration partners for further clinical development.

Reducing Early Risk to Enable Further Development
Oncode Institute supported the development of the original scientific concept and funded its early clinical translation through its Clinical Proof-of-Concept programme (CPoC). This allowed the researchers not only to test whether the vaccine was safe and induced the intended immune response in patients, but also to respond to new scientific insights during the study.
Following the clinical proof-of-concept, Oncode Institute also supported further valorization, from assessment of intellectual property and development strategy, to company formation and the first financing phase.
To make the vaccine as broadly applicable and robust as possible, it needs to contain multiple TEIPP antigens. The so-called “dark proteome” is an important source of tumor-specific antigens that may be used for this purpose. By drawing on the expertise and infrastructure within Oncode Accelerator, and through a Demonstrator Project, additional TEIPPs can be identified and validated for use in the TEIPP vaccine.

Nieuw biotechbedrijf gaat veelbelovend TEIPP-vaccin tegen longkanker verder ontwikkelen na eerste klinische proof-of-concept
Therapeutische kankervaccins staan opnieuw volop in de belangstelling. Kort na de positieve fase III-resultaten van Moderna en Merck met een gepersonaliseerd mRNA-kankervaccin, wordt in Nederland een volgende stap gezet met een andere veelbelovende vaccinstrategie. Het nieuwe biotechbedrijf Revealis Biotherapeutics gaat het zogenoemde TEIPP-vaccin verder ontwikkelen, dat zich richt op tumorcellen die zich aan het immuunsysteem hebben weten te onttrekken. Dit soort kankercellen zijn al in een vroeg stadium aanwezig en vormt één van de redenen waarom immunotherapie later tekortschiet.
Het TEIPP-vaccin is ontwikkeld door onderzoekers van onder meer het Leids Universitair Medisch Centrum en Oncode Institute. In een eerste fase I/II-studie bij 26 patiënten met uitgezaaide niet-kleincellige longkanker wekte het vaccin bij meer dan 80 procent van de deelnemers een specifieke immuunreactie op. De resultaten werden gepubliceerd in Nature Communications en leverden het klinische proof-of-concept om de technologie verder te ontwikkelen.
Nieuw momentum voor kankervaccins
Therapeutische kankervaccins worden al decennialang onderzocht, maar veel eerdere kandidaten wisten een immuunrespons onvoldoende te vertalen naar klinisch voordeel in patiënten met kanker. Door nieuwe inzichten in de afweer tegen kanker, betere doelwitten, combinaties met immuuntherapie en het moment van toepassing verandert dat perspectief.
Moderna en Merck maakten op 19 augustus positieve fase III-resultaten bekend voor hun gepersonaliseerde mRNA-kankervaccin in combinatie met pembrolizumab bij hoog-risico melanoom. De technologie van Revealis Biotherapeutics is anders: het TEIPP-vaccin richt zich op een specifiek mechanisme waarmee tumorcellen aan het immuunsysteem ontsnappen.
Professor Sjoerd van der Burg: “De meeste patiënten gaan niet dood aan de eerste tumor, maar aan diegene die opnieuw terugkomen of zich verspreiden. De recente ontwikkelingen laten zien dat het kanker vaccins werken: je kunt en moet het immuunsysteem gericht leren kanker aan te vallen, je moet de immuunreactie in stand houden met checkpoint blokkade en je moet dit doen op het moment dat de primaire tumor weg is met het doel om nieuwe tumoren te voorkomen. Ons vaccin verschilt wezenlijk van die van Moderna en Merck, maar bouwt op hetzelfde fundamentele inzicht, hierdoor ontstaat een nieuwe therapeutische mogelijkheid.”
Juist de onzichtbare tumorcel aanvallen
Een van de grote uitdagingen van immuuntherapie is dat tumorcellen zich kunnen aanpassen. Sommige tumorcellen veranderen hun antigeenpresentatie en worden daardoor vrijwel onzichtbaar voor de T-cellen die ze zouden moeten aanvallen. Deze tumorcellen kunnen uiteindelijk leiden tot hernieuwde tumorgroei waarbij huidige immunotherapieën niet goed meer werken. De technologie achter Revealis Biotherapeutics richt zich op het aanvallen van deze ontsnapte tumorcellen om de immuunocontrole te herstellen.
Onderzoekers onder leiding van Sjoerd van der Burg en Thorbald van Hall ontdekten dat deze ‘onzichtbare’ tumorcellen tegelijkertijd specifieke TEIPP-antigenen laten zien. Die ontstaan juist wanneer het normale mechanisme waarmee cellen antigenen presenteren verstoord raakt.
Helaas reageert het immuunsysteem niet vanzelf tegen TEIPP antigenen. Het vaccin leert het afweersysteem om wel deze TEIPP-antigenen herkennen en probeert zo een ontsnappingsmechanisme van de tumor in een kwetsbaarheid te veranderen.
In de eerste klinische studie werd aangetoond dat dit principe ook bij patiënten werkt: bij ruim 80 procent ontstond na vaccinatie een TEIPP-specifieke immuunrespons.
Van wetenschappelijk inzicht naar bedrijf
Revealis Biotherapeutics gaat een volgende generatie van het vaccin ontwikkelen met meerdere TEIPP-peptiden. Daarmee wil het bedrijf een bredere afweerreactie kunnen opwekken en de technologie geschikt maken voor een grotere groep patiënten.
Voor deze eerste ontwikkelfase is pre-seedfinanciering bijeengebracht door Oncode BV en twee family offices. Revealis Biotherapeutics zal voor verdere klinische ontwikkeling op termijn aanvullend kapitaal en samenwerkingspartners nodig hebben.

Risico vroeg verkleinen zodat ontwikkeling verder kan
Oncode Institute ondersteunde de ontwikkeling van het oorspronkelijke wetenschappelijke concept en financierde via zijn Clinical Proof-of-Concept-programma (CPoC) de vroege klinische vertaling. Daardoor konden de onderzoekers niet alleen testen of het vaccin veilig was en daadwerkelijk een immuunreactie bij patiënten opwekte, maar ook gedurende het onderzoek reageren op nieuwe wetenschappelijke inzichten.
Na het klinische proof-of-concept ondersteunde Oncode Institute ook de verdere valorisatie: van beoordeling intellectueel eigendom en ontwikkelstrategie tot het opzetten van het bedrijf en de eerste financieringsfase.
Om het vaccin breed toepasbaar en zo sterk mogelijk te maken moet het verschillende TEIPP antigenen bevatten. Het zogenoemde “dark proteome” is een belangrijke bron voor tumor-specifieke antigenen die gebruikt kunnen worden. Door gebruikmaking van alle expertise binnen Oncode Accelerator, en het verkrijgen van een Demonstrator Project, kunnen ook dit type TEIPPs geïdentificeerd worden voor toepassing in het TEIPP vaccin.
